I am an In-Network Provider for First Choice and Kaiser PPO Plans. I am an Out of Network Provider for all other insurances. I am able to provide a super-bill for you to submit to your insurance for reimbursement.  Please check with your insurance company to determine your out-of-network benefits. It is likely your insurance company will cover a portion of the cost.

When speaking with your insurance provider, here is what to ask:

   *What is my deductible for out of network mental health outpatient services? Has any of that deductible been met?
   *What is the insurance's reimbursement rate for an out-of-network mental health provider?
   *What is my coinsurance percentage? 
  *Where can I find the form necessary to submit my own medical claim for reimbursement? 
   *What address or email do I send this form?

Individual Adult Therapy:  My rate for 50 minute sessions is $180, Initial Intake Appointment $200
* You may request a free complementary 25 minute meet & greet session to ensure we are a good fit.
Payment: Payment is due at the beginning of each session; unless otherwise negotiated.

Cancellations: Please provide 24 hours notice if you need to cancel or rescheduled an appointment.
Late cancellations with less than 24 hour notice will be charged the full session fee.
